Volunteer Vehicle Usage

ALL VOLUNTEER DRIVERS FOR SPORTING EVENTS MUST COMPLETE THE VOLUNTEER VEHICLE USAGE FORM PRIOR TO TRANSPORTING ANY STUDENT-ATHLETES.  ALL FORMS ARE DUE TO THE ATHLETICS OFFICE
 

Steps to follow

Step 1

Complete the Volunteer Vehicle Usage Form which is available in "Forms" on this page or picked up from the athletics office.

Step 2

Attach a copy of your driver's license

Step 3

Attach a copy of your current vehicle registration

Step 4

Attach a copy of your auto insurance card and/or policy with minimum required limits

Step 5

Complete the Volunteer Vehicle Usage Form which is available in "Forms" on this page or picked up from the athletics office.

Obtain Fingerprinting Clearance form from the Athletics office and set appointment.  There is a fee of $25 and fingerprinting is done at the district office with appointment

Step 6

Attach a copy of your driver's license

Complete the DMV Pull form which is available in "Forms" on this page or picked up from the Athletics office
